Cancer and Advance Care Planning
We are pleased to present two new advance care planning tools.  These tools – one for patients and families and one for professionals – are specifically designed for patients with a cancer diagnosis.  They were developed as a partnership between the Advance Care Planning in Canada initiative, the BC Cancer Agency and members of the BC Patient Voices Network.
